A vacancy has arisen for a transition co-ordinator. The successful candidate will support the transition of pupils from Bower Grove to their 16+  destinations and ensure pupils establish themselves into the next phase of their education. The position is for 37 hours per week, term time only (39 weeks).

The post holder will deliver and support pupils learning in the most effective way, working with multidisciplinary teams, working in colleges and training providers and/or employers’ facilities, using behaviour management programmes to meet pupils’ complex special educational needs.    

The post holder will need excellent communication and reporting skills and good IT skills. Significant experience of working with Key Stage 4 pupils with SEMH needs is essential, along with willingness to travel between 16+ providers and on occasion take pupils in their own vehicle. Confidence to undertake home visits if necessary and to work with a high level of independence and usually until 4.30pm daily. The successful applicant must have a full driving licence and be a car owner.
